The Summer Spine Golf Tournament and Day at the Range: Trap Shooting & Archery Challenge will be competitive events in the Summer Spine Symposium Challenge Cup. Participate to win points and glory for your home program!
Horseback Riding is a non-competitive activity. Mountain Biking has been cancelled due to low enrollment.
Payment confirmation will be provided upon submission of this form. A unique confirmation email with details for each activity type will be sent separately in advance of the meeting.
Please note that any activity participants under the age of 18 years must be accompanied by a registered adult for the duration of the activity.

SUMMER SPINE GOLF TOURNAMENT

Open registration for Golf closed on June 14; however, there are still open spots available in our guaranteed minimum. Registrations will continue to be accepted up to 20 players.

All golfers in attendance are invited to participate in the Summer Spine Golf Tournament, even if you're not an alum of Cedars-Sinai Spine or Och Spine hospital. In addition to awarding the banner of victory to the winningest spine program, recognition will also be awarded for challenge holes and best scoring players. Questions? Contact Alison Matta.

Time: 10:45 am shotgun start; conclude by 3:00 pm
Location: Canyons Golf at Grand Summit (onsite, no transportation required)
Deadline: Sign up by June 14, 2024 to guarantee your spot.

Sandwiches and drinks are available for purchase on-course. Water is provided.
No cancellations are permitted after June 14, 2024. Pairing will be announced in advance of the meeting.

DAY AT THE RANGE

Open registration for Day at the Range closed on June 14; however, there are still open spots available in our guaranteed minimum. Registrations will continue to be accepted up to 20 attendees.

Join your fellow Challenge Cup participants in an Archery and Trapshooting Round-Robin event at North Forty Ranch where you can test your aim for the glory of your spine program. Bonus! Enjoy a freshly-smoked BBQ meal prepared as you arrive for a full high west experience.

If the required minimum of 10 participants is not met, others from outside the program may be included in this activity. Shuttles from the hotel will remain private to the group.
