The Summer Spine Golf Tournament and Trap Shooting Challenge will be competitive events in the Summer Spine Symposium Challenge Cup. Participate to win points and glory for your home program!
Fly Fishing and Whitewater Rafting are non-competitive activities.
Please note that all four activities conflict with one another—all take place starting around 10:30 am on Friday.
Payment confirmation will be provided upon submission of this form. A unique confirmation email with details for each activity type will be sent separately in advance of the meeting.
Please note that any activity participants under the age of 16 years must be accompanied by a registered adult for the duration of the activity.

SUMMER SPINE GOLF TOURNAMENT

All golfers in attendance are invited to participate in the Summer Spine Golf Tournament, even if you're not an alum of Cedars-Sinai Spine, Duke Spine, or Och Spine hospital. In addition to awarding the banner of victory to the winningest spine program, recognition will also be awarded for challenge holes and best scoring players. Questions? Contact Alison Matta.

Time: 10:45 am shotgun start; conclude by 3:00 pm
Location: Canyons Golf at Grand Summit (onsite, no transportation required)
Deadline: Sign up by July 1, 2025 to guarantee your spot.

Sandwiches and drinks are available for purchase on-course. Water is provided.

TRAP SHOOTING CHALLENGE

Join your fellow Challenge Cup participants in a trap shooting event hosted by All-Seasons Adventures (our 2023 vendor) where you can test your aim for the glory of your spine program.

FLY FISHING

Fish with an experienced guide and exclusive access to rivers, streams and lakes in the Uinta National Forest. Angle for Grayling and Brook trout as you enjoy idyllic mountain views. We will handle getting your license, gear, and all the terminal tackle for you. Transportation and lunch included.

WHITEWATER RAFTING

Bring the whole family on an expertly guided rafting adventure on the Weber River. With the first rapid only 50 yards below the put in, this excursion makes for an exciting half-day trip on a 13-foot Super Puma raft. Paddle through splashy, exciting Class II rapids with assistance from your experienced guide. Calm stretches provide opportunities to bask in the sun, search for wildlife, or start a water fight.
